iPad Request
For Faculty Members

Please Note: The admissions committee will reserve the iPads during part of November and February. The clinical skills lab program may reserve a number of iPads periodically during the academic year. A member from ASA Ed Tech Team will inform you if iPads are unavailable upon request.

There is a small quantity of iPads for faculty use. Please review the policy, then place your request below:

  • iPads will only operate when connected to CVM Wifi Network.
  • Faculty must give advance notice of two business days prior to check out.
  • Faculty members are given a two week time period with the iPad.
  • Faculty must request extension on the loaner iPads by emailing Sarah Bell
  • Faculty members will be notified by a staff member when iPad is available for scheduled pick up.
  • iPads must be picked up and returned to ASA Ed Tech Team during business hours: M-F, 8AM – 5PM.

iPad Request Form

  • We use your UF ID Number to scan the iPad (and materials) under your university identification. This is a library policy.
  • We need your university email to confirm when your reservation is complete.
  • You get two weeks with the iPad. Input the date you wish to check out the iPad.
  • Why do you need the iPad? This information helps OSI to collect data on how iPads are used in the curriculum.
